Consistent FPS drops

This has happened since the OB update, I just haven't bothered doing anything about it until now.

My FPS is good (usually 200-300 in a 5on5 game), but maybe once or twice every second (literally) it drops to around 60 and then straight back up again. The game feels really laggy all the time, and "smoothness" is pretty much gone. Even walking around the maps has a kind of out-of-sync feeling, especially compared to old source. My lerp also flashes orange and yellow quickly. I tried it on a local server ("create a server" from the menu) so it certainly isn't my internet or router (save that for another thread, they're pretty shit too).

I formatted yesterday, but it does the exact same thing. I have also reinstalled source, and deleted my cstrike and tried playing on a default config, but nothing has helped :(

Are my net settings ok etc? Are there any commands or settings I should be using?

I'm on windows XP, using an ati 4890 with latest drivers and an intel i5 cpu, if that makes a difference.
